Three Categories
Vulnerable populations can be divided into three categories: physical, psychological, and social. These patient cohorts often lack alternative resources, though not traditional to healthcare delivery systems, they contribute substantially to treatment effectiveness, outcomes, and overall cost.
Top-Tier Reimbursements
Resolving vulnerabilities reduces cost, improves outcomes, increases patient engagement and loyalty. It helps qualify the provider organization for top-tier reimbursement while better supporting the community in which it operates.
Social Determinants of Health
Healthy People 2020 focuses on social determinants of health as a way of identifying people at risk of poorer health outcomes. They group these determinants into five categories: economic stability, education, social and community context, health and health care, and neighborhood and built environment. Our system provides a simple and effective methodology to collect data around SDOH and analysis tools that allow for intervention implementation.
